Food allergy checklist

Having information about the food allergies affecting staff or service users enables you to implement risk reduction measures and prepare for an emergency. Use our form to gather initial information.

Allergies

Our Food Allergy Checklist can be issued to staff or service users to collect information about any food allergies they may have and the medicines they carry.

Using such a questionnaire is not compulsory, but It’s helpful if you provide food as part of a service, e.g. if you provide food to employees and others in the workplace.

As a minimum we recommend you issue it on a case by case basis, i.e. if you are alerted to a risk of a serious allergic reaction in an individual.

The manager should review the information before signing. They should liaise with others such as first aiders to ensure that an appropriate emergency response is prepared and take such other action as might be appropriate to reduce the risk of a dangerous allergic reaction.

The form should be filed confidentially once the contents have been understood by those responsible for food provision and first aid response.
