Contact tracing support form

To support the NHS Test and Trace strategy, some organisations are being asked to collect customer and visitor details. If this applies to you, use our form to gather the information.

Our form

Our Contact Tracing Support Form is to be used where customers and visitors spend a long period of time on the premises with the potential to come into close contact with others from outside of their household. It’s been designed based on the instructions in the government’s guidance.

Who does it apply to?

You should use the form if you are in the following sectors, whether using an indoor or outdoor venue, or a mobile setting:

- hospitality, including pubs, bars, restaurants and cafés

- tourism and leisure, including hotels, museums, cinemas, zoos and theme parks

- close contact services, such as hairdressers, barbershops and tailors

- facilities provided by local authorities, including town halls and civic centres for events, community centres, libraries and children’s centres

- places of worship, including used for events and other community activities.

Note. The requirements do not apply when services are taken off site immediately, e.g. during a takeaway collection.

What to collect

Although customers and visitors are not obliged to give their details you should encourage them to do so. It is important that as many customer/ visitor details as possible are collected to assist the NHS should the data be needed.

However, you should not ask for more information than we’ve covered in our form. All data collected is personal information and therefore must be handled in accordance with data protection rules.

The information collected is only to be held on record for 21 days, after which time it should be securely disposed of or deleted.

Note. You must communicate to customers and visitors why the data is being collected and how it may be used. You might wish to display a notice at your premises to explain this.
