Request for change or modification

Its becoming increasingly important to manage changes so that there are no negative impacts to your business. Our form provides you with a detailed record of what’s been changed or modified, why and when.

What’s covered?

Section 1 of our Request for Change or Modification form should be completed by the person proposing the change. It asks for justification of the change, whether it is a minor or major piece of work, and which departments may be affected. They r then sign and pass it to their manager so they can discuss and approve. If there are no negative changes the form can be closed at this point once authority has been given. There is a prompt to review the change by a certain date to assess whether it has been successfully implemented.

If the change is major and impacts other areas such as quality, compliance, etc. you are prompted to complete Section 2, which asks for more detailed risk assessments. There is also provision for more cross-functional roles to be involved in the change. The form should be authorised by each manager concerned.

Note. Only when the form is fully authorised should the change or modification proceed.

When to use the document?

Complete a form each time a change or modification is required. You will then have a formal record and can produce it in the event of something going wrong such as mechanical failure or an accident. Keep the document for a minimum of three years or the lifetime of the equipment being modified.
